      Gather documents proving your citizenship (such as a birth certificate or passport) and identity (identification with a photograph such as a driver's license or passport). Immigrants must have documents proving identity, immigration status, and work eligibility.

      Print and fill out an Application for a Social Security Card from the Social Security Administration website.

      Mail or bring your application and documents to the Raleigh Social Security Office at 4701 Old Wake Forest, Raleigh, NC 27609. You will receive your replacement card and your sent documents in 1 to 2 weeks.
