How Can I Obtain a Copy of My CNA License?

Every state requires that a Certified Nursing Assistant (CNA) register within the state of her employment. If you are a Certified Nursing Assistant and would like a copy of your original certificate, the information is easy to obtain by contacting the right agency.

Locate the contact information for your state's CNA registry by clicking through the Nurse Aide Registry website. By contacting your state's registry, you can change your name, move to another state, renew your CNA certification, get nurse aide test information and more.

Verify your identity and certification with your state's registry. Most states allow you to verify your certification over the phone and a few states provide verification only over the phone. Follow the prompts, whether you apply for your license copy over the phone or online.

Provide your Social Security number and/or certification number to your state's registry.

Pay any required fees. Most states require a fee (which varies from state to state) to reissue a copy of your CNA license.
