Things You'll Need

• Birth certificate or passport
• Driver's license or other proof of identity
• Legal document showing your old and new name

Instructions

1. Print out or complete the proper SS-5 form. You can find this online or at your Michigan local social security office.

2. Show proof of your identity. In Michigan, you may use your driver's license, passport or non-driver identity card. You must also show proof of your age, such as a birth certificate or passport.

3. Display recent legal proof of your name change. This must be a legal document showing your old and new name change within the past two years. If the change was less recent, you must provide other documentation that shows your old and new name.
