How to Register Trade Names in Utah

If you own and operate a business in the state of Utah under a trade name that is different from the company's legal name, then it is an assumed name or a "doing business as" (DBA) name. The legal name of a business is the name of the business owner, or owners, in a sole proprietorship or general partnership. If the business is a corporation, limited liability company, limited partnership or limited liability partnership, then the company's legal name is the name that is listed on the organizing document filed with the Utah Division of Corporations and Commercial Code. All DBA trade names must be filed with the Division.

      Obtain a "Business Name Registration/DBA Application." You can download the form from the Utah Division of Corporations and Commercial Code website or you can request a copy from the Division by contacting it at: 801-530-4849.

      Complete the "Business Name Registration/DBA Application." The form must be type-written or completed on a computer and printed. In order to complete the form, you must provide the following information: the name that you are requesting for the business, the purpose of the business, the business's address, the name and contact information of the registered agent, and the names and contact information of the business owners.

      Submit the "Business Name Registration/DBA Application" with the filing fee. The required filing fee for this application is $22. You can mail the form to: Utah Division of Corporations & Commercial Code, PO Box 146705, Salt Lake City, Utah 84114-6705. You can submit the form in person to: Heber M. Wells Building, 160 E 300 S, 1st Floor, Salt Lake City, Utah 84111. You can also fax the form to the Division at: 801-530-6438.

The website is self-guided and will prompt you to answer questions regarding your DBA filing. You will also have to create a personal account and provide credit card information to pay the filing fee.
