Social Security was signed into law in 1935 as a part of President Roosevelt's New Deal. The Social Security Administration is now the largest government agency in the world. All United States citizens are required to have a social security card. If your last name changes through marriage, you must apply for a new social security card with the Social Security Administration. Your new card will have the same number but show your new name on the front.

Instructions

      Fill out Form SS-5, Application for a Social Security Card. The application is available on the Social Security Administration (SSA) website and at your local SSA office.

      Find your local office by going to the SSA website and putting your zip code into the office locator box. The offices nearest your home will be shown on the screen.

      Go to your local office. Bring your SS-5 Application and proof of your name change, such as your marriage certificate. Be sure to bring photo identification that clearly shows your former last name on it.

      Receive your new card in the mail. Your new card is mailed once your documents have been verified by the Social Security Administration as valid. Your new card will have the same number with your new name on the front.
