How to Stop XBox Mods
Xbox Live is an online community that allows gamers from every corner of the world to connect in multiplayer games and compete with one another. Unfortunately, a small percentage of these gamers choose to use Xbox Live as a sort of “hacking” playground, and modify their Xbox consoles and games to provide them with an unfair advantage or cause harm to the gameplay of others. While you may not be able to stop other gamers from modifying their consoles directly, you can play a vital part in the fight against "mods" by reporting these players to Microsoft through the Xbox Live report system. If a player is found to have violated the Live terms of service, their account could be terminated and their access lost.

Open the player’s gamer profile. Here you will see their information, community rating, and other information.

Select “File Complaint” and press the A button.

Choose “Tampering” as the general reason for your report, and on the following menu select either “System Tampering” or “Cheating.”

Select “File Complaint” to complete the process. The Microsoft Xbox Live team will investigate your report and take the appropriate actions based on their findings.

Tips & Warnings
If you encounter a cheating player, file a complaint and leave it at that. Taunting, threatening, or any other sort of interaction with them will not help with the investigation process and will only cause unnecessary drama.
Be aware that connection issues or skill differences can often make it seem like other players are cheating. Be sure that you are reporting real cases of abuse!
Do not abuse the Xbox Live complaint system by reporting players you have personal conflict with or don't like. The system is in place to protect the Live Terms of Service, not to carry out vendettas.
