There are a number of things you may need a certified copy of your birth certificate for, such as obtaining a passport or proving residency. If you don't have a copy of yours, don't worry; the process of obtaining your birth certificate is not as painful as you might think. As long as you are prepared with some basic information and documentation, there are ways you can obtain your Oklahoma birth certificate.

      Fill out the application form. You will need your full name at birth, the date and place of birth. The form also requires the full names of both your parents, your relationship to the person on the certificate and a current phone number.

      Present a government issued form of identification and pay the current application fee. You will need to plan on up to an hour to obtain the requested certificate.

      Fill out the Application for Search and Certified Copy of Birth Certificate. This can be accessed online at: http://www.ok.gov/health/documents/VR_BCRequest_interactive.pdf

      You will need your full name at birth, the date and place of birth. The form also requires the full names of both your parents, your relationship to the person on the certificate and a current phone number.

      Include the application fee and a copy of a government issued form of ID (do not include the original).

      State your relationship to the person whose certificate you are requesting, and sign the form.

      Certified mail is an ideal option if you want to make sure your application arrives and is received. Allow up to four weeks for processing.

      Submit your financial information to pay the application fees as well as the expedition fees if requested. All major credit cards are accepted, as well as debit options.

Tips & Warnings

  • Acceptable forms of ID include: a state-issued drivers license or other state-issued ID, a US passport, a tribal membership card, an Oklahoma gun permit, or a pilot's license.
