How Do I get a Replacement Social Security Card in NYC?
You can replace your Social Security card up to three times in a year and 10 times in your lifetime. However, the Social Security Administration will not count replacements for legal name changes and will allow more replacements if you need it to help with a significant hardship. New York City residents who are older than 12 can apply for a replacement at a local office or via mail. Applicants who are 12 or younger must apply in person, accompanied by a guardian.

Complete an application for a Social Security card. You must complete the application and select relevant options that indicate you are applying for a replacement. Download the form from the Social Security Administration website or obtain it from your local New York City Social Security office (see Resources).

Gather proof of your identity and U.S. citizenship or legal residence. You can present items such as your driver’s license, New York state ID, passport, birth certificate or permanent residence card. These must be original documents or copies that have been certified by the issuing agency.

Submit your completed form and supporting documents to your local Social Security office via mail or in person (see Resources). The office will process your application and send your replacement card via mail.
