How to Complete a W4 Tax Form
Form W-4 notifies an employer of how much to withhold for taxes from an employee's paycheck each time the employer issues the employee a paycheck. The Internal Revenue Service recommends filling out a Form W-4 each year. If the taxpayer does not fill out Form W-4 each year, then he should try to fill out the form whenever his financial situation changes in any way.

Complete the Personal Allowances Worksheet attached with Form W-4 on Page 1. Follow the instructions provided on the worksheet. You will need to fill out letters A through H on the worksheet. The results of this worksheet will vary on your filing status, job status and number of dependents.

Enter your first name, middle initial last name and address on Form W-4 Line 1. Enter your Social Security Number on Line 2.

Check if you are filing as single, married or married filing at a higher rate for Line 3.

Enter the amount of allowances from the Personal Allowances Worksheet in Step 1 on Line 5.

Enter any additional amounts you would like to be withheld from your pay.

Enter "Exempt" in Line 7 if you are exempt from withholding. To be exempt, you must have no tax liability from the previous year and you must not expect to have tax liability this year.

Sign and date Form W-4.

Write your employer's name and address on Line 8 and the employer identification number on Line 10.
