Tax fraud is a very serious federal offense.

The Internal Revenue Service has made it easy to report suspected tax fraud. The IRS has a Whistleblower Office to reward people who report information that allows the agency to collect past-due taxes, penalties and interest. The information provided to the IRS has to be specific and documented. If the IRS proves fraud, the whistleblower could be rewarded with 15 percent to 30 percent of the money the IRS collects.

      Obtain IRS Form 3949 A and complete it. You will need detailed information about your employer such as company name, address and Employer Identification Number, all of which can be found on a W-2 form you received from your employer or your paycheck stub. You will also need to provide specific information about the suspected fraud. The form asks for your name and address, but this is optional.

      Write a letter instead of using Form 3949 A. Include the name and address of your employer, your employer's EIN, a description of the suspected fraud and how you know about it, what years the fraud took place and how much income you suspect went unreported.

      Make a copy of the form, or your letter, and mail the original to: Internal Revenue Service, Fresno, CA 93888.
