Birth certificates are used as documentation of citizenship and proof of identity that can be used to get drivers' licenses and passports. Illinois issues birth certificates from the local county clerk's office. Adults may request their own birth certificate or a copy of a minor child's when your name appears on the birth certificate. Proof of court appointed legal guardianship is required to obtain a birth certificate if the adult's name does not appear on the certificate.

Instructions

      Complete the birth certificate request form from the Cook County Clerk website. Fill out all information. Chicago is in Cook County.

      Photocopy your driver's license or state ID card. Issue a check or money order to Cook County Clerk for $15 for the first birth certificate copy and $4 for each additional copy requested.

      Mail the completed form with the copy of the photo identification, the check or money order and a self-addressed stamped envelope to:

      The birth certificate will be mailed to you in the envelope.

Tips & Warnings

  • Birth certificates can also be requested over the phone at 866-252-8974, or in person at the Cook County Clerk's office or a local currency exchange office. A $5 fee is added if the certificate is requested at a currency exchange office.
