How to File a DS-230 Form for Newborn Babies
The DS-230 form is required for immigrants who are seeking to obtain a visa for legal travel within the United States. Regardless of age, a form must be completed for all family members who are planning to travel within the U.S. As a result, newborn babies are also required to have a completed DS-230 form to enter the country. The DS-230 form has two parts and contains questions regarding specific biographical information required to obtain an immigrant visa.

Download the application form for an immigrant visa and alien registration. Visit Travel.State.Gov for the DS-230 application form and instructions. Download both Part 1 and Part 2 of the DS-230 form. To download Part 1 and Part 2 of the form, you will need to have a recent version of Adobe Reader installed on your computer (see Resources). Downloading a current version of Adobe reader is free. You will receive a "file corrupted" error message if you are using a version of Adobe Reader that is too outdated.

Complete Part I of form DS-230 for yourself and each member of your family. Complete a form for every family member, regardless of age, who will traveling with you, including newborn babies. Print or type your answers to all questions. Mark questions that are not applicable with the letters "N/A." Answer on a separate piece of paper if there is not enough room to answer on the form. Attach these additional sheets to the DS-230 form.

Complete Part 2 of the DS-230 form. This form is the sworn statement that testifies that the information you provided is accurate. Answer all questions on Part 2, but do not sign the form.

Sign the bottom of page 2 on the DS-230 form Part 1. Sign the form for minors who are younger than 14. Do not sign Part 2 on any of the forms until you are instructed to do so at the consular office. Make a copy of both parts for your personal records.

Send the DS-230 Part 1 forms for all family members to the National Visa Center in Portsmouth, New Hampshire. Send the forms using certified or registered mail and include a return receipt. Keep all Part 2 DS-230 forms with you. Take these forms to the consulate office when you have been given instructions for the final interview. You will be asked to sign Part 2 while you are there.

Do not leave any fields blank. For questions that do not apply to you, use the N/A abbreviation. Use this same format when filling out the forms for a newborn baby.
Do not falsify information on the DS-230 form. Any false statement or concealment of a fact could result in permanent exclusion from the United States.
