Changing your name in Queensland is possible if you were born in Queensland or if you have lived in Queensland for 12 months and were born overseas. Queensland's requirements to change your name are complex and will require thorough reading of the application to ensure you follow the directions.

Instructions

      Fill out the Queensland name change form. You can download the form from the Queensland Government Department of Justice and Attorney-General website (see Resources). Complete the form in full, print it, and have it signed in the presence of a notary.

      Provide proof of identity. Copies must be certified. You will need to provide proof of identity, proof of operating in the community and proof of residence. View the list of accepted forms of proof on the government website (see Resources).

      Provide certified proof of any previous name changes, marriages or divorces. This documentation will help Queensland establish the link between your old name and your new requested name.

      Submit your application to the address shown on your name change form. Pay the fee required. As of June 2010, the fee is $135.00. If submitting your application in person you can pay with cash, cheque or money order. If mailing in your application, payment can be made with cheque or money order.

      Allow four to six weeks for the Queensland Government to approve of your name change. You will be contacted if any additional documentation is required. You will be notified of any court appearances required. If you need to contact Queensland regarding your application, make sure you have the date you submitted the application, your method of payment, a reference number and a copy of your application.

      Notify all government agencies, creditors, physicians and anyone else you do business with of your name change. Certain agencies will require a copy of the signed court order.
