      Visit a local Social Security Administration office. If you do not know where the nearest office is, click on the link in the Resources section, type in your zip code and look at the results page for a map to the nearest SSA facility.

      Inform a Social Security Administration representative that you want to get your W-2 form from a specific year in order to resolve an issue with SSA.

      Complete an application to receive the W-2 form by providing information like your name, address, Social Security number, birth date and the tax year of the W-2 form that you need in order to resolve your SSA matter. Sign the bottom of the form and hand it, along with your state-issued photo identification card, to the representative. This is a free service.
