How to Apply for a Permanent Resident Card in Canada
A Permanent Resident Card (PR Card), according to Citizenship and Immigration Canada (CIC) "is a status document that provides secure proof of permanent resident status." If you recently became a Canadian permanent resident, you must apply for this document from CIC. CIC expects permanent residents to account for their PR Card at all times. In fact, CIC will not issue a replacement PR Card if you do not return or account for a previously issued Permanent Resident Card. Therefore, handle your PR Card with utmost care.

Download Form IMM 5444, Application for a Permanent Resident Card from the homepage of Citizenship and Immigration Canada (see Resources).

Order Forms IMM 5401, HPM Receipt and IMM 5455, Supplementary Identification Form by visiting the "Permanent Resident Card (PR Card)" page (see Resources). Go to the "Order Information" section of the page, complete the questions and then select the "Submit" button.

Complete all the forms as thoroughly as possible. Date and sign each form.

Gather supporting documents for your application, which includes a copy of your valid passport or the passport you held when you became a permanent resident and a copy of your Record of Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292).
If you have ever changed your name, provide certified copies of your legal name change document(s) as well as a certified copy of your provincial driver's license. If you are an applicant who is under 18 years of age, provide a copy of your birth certificate or legal documentation providing details about your guardian(s).

Have two color, or black or white 35 mm by 45 mm photographs taken of you within 12 months of your application. The back of the photos must clearly display the date when you took the photos and not when you had the photos printed.

Place on of your photos in the marked space provided on Form IMM 5455; do not clue or staple the photo. Place the remaining photo in a small envelope.

Pay the processing fee for your request by completing Form IMM 5401 and bringing the completed form with you to one of the approved financial institutions, as indicated in the form's instructions. As of May 2010, the fee was $50 per person. Upon paying the correct processing fee, the bank teller at the financial institution will authenticate your Form IMM 5401.
