How to Obtain Missouri Divorce Records

The Missouri Department of Health and Senior Services offers those who were divorced in the state of Missouri access to divorce records. If you need a copy of your divorce records, the Vital Records Office will process your request. In order to request these records, you need to complete an application form. You also must submit payment for processing fees.

      Download the "Application for copy of Statement Relating to Marriage or Divorce" located on the Missouri Department of Health and Senior Services website, linked in the Resources section. Downloading the file requires Adobe Acrobat Reader to be installed on your computer.

      Print the application form. Visit your library if you do not have a printer at home.

      Complete the "Application for copy of Statement Relating to Marriage or Divorce" form with accurate information. Indicate you are requesting divorce records. Fill out the "Divorce" portion of the form, on the right side. And remember to fill out your contact information.

      Prepare a self-addressed, stamped envelope.

      Prepare a check or money order for the processing fee, which, as of June 2010, is $15. Make the check or money order payable to the Missouri Department of Health and Senior Services.

      Mail the completed form, self-addressed, stamped envelope and payment to the following address:

  • You may obtain divorce records in person by taking your completed application form and payment to a local health department. The Department of Health and Senior Services website has a list of locations.
