      Log onto the State of Wisconsin website (http://wi.gov/state/index.html) to reserve the name of your corporation. Reservations are $10 for 120 days. Submit Form 1 to reserve your corporation's name. Corporation names must be unique. Requests for names that are already registered are rejected. You can search for registered corporation names using the state's online database.

      Complete the Articles of Incorporation Non-Stock Corporation form. You will need your corporation's name and registered agent information. You will also need to state the purpose of your corporation and list your incorporators.

      Submit two copies of your application along with a $35 filing fee to:

      Madison, WI 53707

      Make your check payable to the Department of Financial Institutions. Applications sent by express mail need to be sent to:

      You will receive your file-stamped application by mail. Typical processing time is five days.
