How to Set Up a DBA in Arizona

In Arizona, setting up a DBA is an optional but commonly accepted practice.

Setting up a DBA in Arizona involves registering your trademark, or company name, with the Arizona Secretary of State. When deciding what to name your business, it's best to use the state's Registered Name Information Search (see link in the Resources section) to ensure you choose a name that no one else has already registered. Wait until you receive your registration certificate before you use your chosen name in an official capacity.

Instructions

1. Download the "Application for Registration of Trade Name" form (see Resources for link).

2. Type your responses into the form, which is an interactive PDF. Complete filling out the form in one sitting, as you cannot save your responses and return to the form later. Print and fill out a blank copy by hand if you prefer.

3. Bring the form to a notary public and sign it in front of her. Do not sign it ahead of time.

4. Send the application and payment to the following address:
