      Download form DO-21 from the New Jersey Motor Vehicle Commission website (see Resources) if you want to get a copy of your driving record by mail or in person. The form is the "Driver History Abstract Request" form. Alternatively, you can pick up a copy of the form at your nearest New Jersey Motor Vehicle Commission service center. Complete the form with your personal information.

      Submit the form in person at your local New Jersey Motor Vehicle Commission service center or by mail. A copy of your driver's license is required if you submit your request by mail. Also be sure to include the fee, which is $15 as of May 2010. Mailed requests should go to:

      Request your New Jersey driving record online (see Resources) if you want to see it immediately. The online request has an additional $10 convenience fee in addition to the standard $15 fee for a copy of your driving record.
