Create a survey using Survey Monkey. Create an account to start crafting surveys for your customers in this browser-based platform. You can use the free Basic plan or a paid Pro plan (best if you plan to send many surveys to your clients). Once you create your customer service survey, email it to your customer list. When you get responses, Survey Monkey allows you to view and analyze the results in your online account.

Download a survey template from Microsoft Office online if you have Microsoft Word on your computer. Modify the form template to your needs—you can add your own company logo and information as well as specific questions to the form. This option is ideal if you plan to send printed surveys to your customers or an attachment via email.

The customer can simply click the link (provided by the service), fill out his answers, and you'll receive the survey results in your online account. The only catch is that you can only receive up to 50 replies and the survey is only active for 10 days, so only send it to your top customers.. You can add 20 questions, which is enough to get a clear idea of the opinions of your customers.
