      Contact your accountant or tax preparation agency and request a copy of your 2004 tax return. They may be able to provide you with the copy they have in their records.

      Contact the website that electronically filed your return. Go to the website and log in using the login identification you created when you e-filed your return. Select the option to view previous tax returns. Select the year "2004" and print the return.

      Submit Form 4506, Request for Copy of Tax Return (See Resources) to the IRS. You need to provide your identifying information: name, tax ID number and address. In addition, you need to indicate that you are requesting the 2004 tax return. Pay the fee. As of May 2010, the fee was $57 per tax return. Indicate on your payment method that this is a form 4506 request. Make the payment out to the United States Treasury.

      Mail Form 4506 and payment to the address indicated on the second page of the form. Your specific address will vary depending on your state of residence.

      Wait 60 calendar days for your request to be processed. Contact the IRS at 800-829-1040 after 60 calendar days to check the status of your request.
