How to Replace a Social Security Card in Pennsylvania

You must have a social security card to work for most employers, and you also may be required to present your social security card when applying for other identification information such as a driver's license. If you have misplaced your social security card, the Social Security Administration (SSA) will give you up to three free copies of your card per calendar year, and up to 10 free copies in a lifetime. The SSA has many offices within the state of Pennsylvania to make requesting a new copy convenient.

      Go to your local Social Security Administration office (see Resources for a listing of Pennsylvania SSA offices).

      Fill out the Application for a Social Security Card Form (SS-5). This form will ask you basic questions pertaining to your identity including you name, birth date, address, place of birth, and the names of your mother and father and their social security numbers if you have them. You can download this form at the SSA's website and fill it out before you arrive at the office.

      Show proof of your identity. You must have a state-issued identification card, a military ID, or a U.S. Passport. Your social security card will then be sent to you in the mail.
