How to Get Patent Forms Mailed to You
When you have created an invention or have an idea for an invention, one of the ways to protect your rights to that invention is to obtain a patent for it. A patent is a legal document that is issued by the U.S. Patent and Trademark Office (USPTO) and grants you the exclusive rights to the invention. This prevents others from using your invention for their own profit. You can download the patent forms from the USPTO website, but you can also call the USPTO and request that the forms be mailed to you.

Determine what type of patent you need and the proper form on which to file your patent. There are forms for utility patents, design patents and plant patents. The USPTO receives over 350,000 patent applications each year, and most of those are for non-provisional utility patents. If you're not sure what type of patent you need, you should consult a patent attorney.

Call the General Information Services Division of the USPTO at 800-786-9199, Monday through Friday, 8:30 a.m. to 5:00 p.m. eastern time (as of April 2010). The office is closed on all federal holidays.

Select option 2, which deals with patent information. Press option 5 to speak with a USPTO representative.

Advise the USPTO representative that you are calling to request that patent forms be mailed to you. Tell the representative the type of patent for which you need forms, and provide your name and mailing address. Ask the representative how long you can expect to wait before receiving the forms.
