      Complete Form SS-5. Form SS-5 is the official Social Security card request application. The web page to find this form is located in the Resources section. The application must be submitted in person. Therefore, you must have a hard copy to present to a Social Security representative.

      Gather your documentation. You will need to provide documentation to support the need for a new Social Security number. You must show proof of the extensive fraud against you. The documentation must be originals or copies of forms certified by the issuing agencies. Notarized or photocopied documents will not be accepted.
