The Canadian National Institute for the Blind is a support and information service for Canadians who have vision loss. Twice a year, the organization sends personalized name and address labels to people who donate on a monthly basis. These labels are not available for sale.

Instructions

      Visit the CNIB monthly donation page under "Donations" on the CNIB website.

      On the monthly donation page, select the amount you want to donate every month ($10, $15, $20 or $50), or select "Other Amount" and enter the amount you want to donate every month.

      Enter your title, first name, last name, street address, city, state/province, postal code, country and email address under the "Your Personal Information" section.

      Select the type of credit card to which you want your donation to be billed and enter your card number, verification code and card expiration date under the "Your Payment Information" section.

      Click "Next." Once the CNIB verifies your donation, it will send you your personalized name and address labels.
