How to Change a Name After Divorce in Alberta
A divorce may often generate a great deal of paperwork. Fortunately, reverting back to your maiden name is a straightforward procedure. According to the government of Alberta, if you've taken your husband's name upon getting married, you may simply resume using your maiden name once you are divorced. The only actions you are required to take is to change your name on your Alberta Health Care Insurance Plan. Alberta Health and Wellness offers a form for this purpose.

Go to Alberta's official website (see Resources). Click on the blue link labeled "Programs & Services." Then click on the link labeled "View all Programs and Services." Under the topics list, click on the link labeled "Health."

Click on the link for the Alberta Health Care Insurance Plan at the top of the list. Open the webpage for "Forms" by clicking the appropriate link from the list on the left side of the page.

Scroll down to the form labeled "AHC0107 Notice of Change." Open this form and print it out. You may not fill out and submit the form online.

Provide the information requested on page two of the form. This information includes your full married name, birth date and mailing address. Notice the separate area on the bottom of page two for the name change information. Write in your full married name, personal health number and reason for the name change (divorce). Write your new last name on the line that states, "I want to change my last name to."

Sign the form on page six, date it and provide your phone number. Since you are divorced, you may ignore the area for the spouse's signature. Mail the form to the address given on the bottom, or deliver it to the nearest office. To find an office close to you, call Alberta Health and Wellness.
