How to Complete Performance Evaluations
A performance evaluation is a tool that a manager, supervisor, or instructor uses to brief an employee or student on his progress. The evaluation is feedback that the recipient can use to gauge whether or not he's meeting the requirements of the job or class. As the evaluator, it's important that you fill out the performance evaluation form correctly. This will eliminate confusion and ensure that the other party has a clear idea as to what you think of his work.

Write the employee's full name, title, and job description at the top of the form. This will help clarify the employee's position and duties as you're filling out the form and while you're conducting your review.

Look over the entire evaluation form before you start to fill it out. Take some time to consider how each item relates to the employee you're planning to evaluate. Look over work samples, check the employee's promptness patterns, and review his file, if necessary, to refresh your memory on his performance.

Rate the employee on each of the elements of the performance evaluation. Performance evaluations commonly include questions about the employee's work ethic, the quality and quantity of his work, promptness, knowledge of job duties, ability to communicate, and how he relates with his co-workers. Rate him on a scale from "below average" or "unacceptable" to "well above average" or "outstanding." Some performance evaluations are rated on a numbered scale---read the corresponding descriptions for each number rating before you start filling out the form.

Expand on each section of the review with comments to help the employee understand your ratings. Give feedback on both positive and negative aspects of the employee or student's work. Mention specific incidents if possible. Explain how the employee can improve. Keep comments short and to the point. Provide an overall written review of the employee's performance at the end of the form so that he'll have a clearer understanding of what you think of his work.
