      Visit Iowa's Department of Education official website.

      Left-click on "Adult Literacy." It is located on the left side of the screen, near the top.

      Left-click on "General Education Development," and scroll down to "Forms and Reports."

      Left-click on "GED Request Form." The form will pop up in a new window.

      Fill out the GED request form and print it.

      Mail your GED request form and a check or money order made out to Iowa Department of Education for $5 (as of April 2010) to:
