Make arrangements to obtain your replacement driver's license as soon as possible, if yours is lost.

Instructions

1. Gather your identifying documents. The state of Texas requires you to provide one primary form of identification, such as a valid passport, alien-resident card or U.S. citizen card. If you don't have a primary form of identification, you'll need to provide one form of secondary identification and two forms of supporting identification.

   Acceptable secondary identification includes an original or certified birth certificate, U.S. certificate of birth, or a valid court order indicating a name or gender change. Acceptable supporting identification includes a vehicle title or registration, voter registration card, auto-insurance card or marriage certificate.

2. Locate your social security card, or another document provide you have a social security number. You can use a Medicare or Medicaid card, a pilot's license, college transcript or VA (Veteran's Administration) card. If you do not have your social security card or another acceptable document, visit a social security office to obtain a replacement.

3. Visit a Dallas Driver's License office to request your replacement driver's license. Bring your documents and $10 to pay the state-required fee. The offices are open Monday through Friday, from 8:00 A.M. to 5:00 P.M. (Central Standard Time). There are three offices in Dallas (downtown, east, southwest).
