How to Change the Name on a Social Security Card in New Jersey

The number of divorces and marriages in New Jersey, as reported in the National Vital Statistics Report Provisional Data for July 2009 by the National Center for Health Statistics, has increased from 4,331 in June 2008 to 5,552 in June 2009. With these numbers, it is logical to assume that the number of individuals requesting a name change on their Social Security cards in New Jersey has also increased. The Social Security Administration's New York Region serves individuals from the states of New York and New Jersey.

Instructions

1. Get documented proof of your name change such as a divorce decree, certificate of naturalization, court order for a change of name or adoption papers. You must get a certified copy of this document from the issuing authority; the Social Security Administration does not accept photocopies or notarized copies. The document must display both your old and new names.

2. Download an application for a Social Security card, Form SS-5, from the Social Security Administration website.

3. Read the overview information, print the application and complete the form by hand. Write legibly and only use black or blue ink. Make sure the "Name to be showed on Card" boxes under section one matches the way you would like your name displayed on your new Social Security card.

4. Gather proof of your identity, which may include, but is not limited to, your New Jersey driver's license or ID or U.S. passport. Submit your immigration document, such as your foreign passport displaying your biometric information and photo, and your Green/Permanent Resident Card, if you are not a U.S. citizen.

5. Mail or take your application and supporting documents to your local SSA office. Visit the "Local Office Search" page on the SSA website, type your New Jersey zip code in the "Enter Zip Code box" and then click "Locate" to obtain the address and phone number of your closest Social Security Administration (SSA) office.
