Determine your eligibility. There are certain guidelines to determine your qualification. As a resident of New York, you can determine your eligibility on their webpage (see References). Once you are on the webpage, simply click the link titled "am I eligible" and provide the requested information.

Obtain the application. Certain counties in New York accept online applications. You can determine if you reside in one of the participating counties on their web page (see References).
If you do not wish to apply online, you can either pick up an application at your local Department of Social Services, or request that one to be mailed to you.

Complete the form. The information provided by you to the Department of Social Services will be checked. You may receive criminal prosecution for knowingly supplying the wrong information. Therefore, it is important to make sure you are truthful.

Return the application. If you applied online, simply submit the form. If you received the form via mail or in person at the Department of Social Service, simply mail the form back. After your form is received and reviewed, you will be notified to come in for the interview. Applicants over 60 years of age may request a telephone interview in lieu of an on-site interview.

Receive an approval. Shortly after your interview with the New York Department of Social Services, you will receive either an approval or rejection letter. If you receive a rejection letter, it will list the reason or reasons you were not approved.
