How to Change Your Maiden Name Online on a Social Security Card
A Social Security number is a unique number that is assigned to a person in order to report that worker's earnings to the United States government and to determine if that person qualifies for Social Security benefits. You are unable to get a job or collect Social Security benefits without a Social Security card. In addition, many credit companies and banks require a Social Security card when opening a new account. If you change your name, you must apply for a new Social Security card.

Fill out Form SS-5, the Application for Social Security Card. You can print the form from the Social Security Administration's website, visit a local office in your area or call the Social Security Administration at 1-800-772-1213 to have one mailed to you.

Show proof of legal name change. If you are newly married, a recent marriage document will suffice.

Show proof of identity. You will need a driver's license, state-issued identification card or U.S. passport for identification purposes. If you don't have one of these or can't get a replacement within 10 days, there are other forms of identification acceptable (see reference).

Bring or mail your application, marriage document and proof of identity to your local Social Security office.

Get a new card. Once your documents have been verified, you will be sent a corrected card in the mail. The card will have the same number but your new name will be listed.

Tips & Warnings
All documents that you give to the Social Security office must be originals or certified copies. They do not accept notarized copies or photocopies.
You must mail your application or take it to your local Social Security office. You cannot apply for or make changes to a Social Security card online.
