      Obtain an application for a Social Security card from either your local Social Security office or by accessing the U.S. Social Security Administration website and clicking on “Social Security Card Application.” Choose “Name Change,” and then click on the “Application for a Social Security Card” to print the application from your computer. You may also obtain an application by calling the Social Security Administration at 800-772-1213.

      Complete the application using blue or black ink only. You must answer all questions with the exception of the questions regarding the mother's and father’s names and Social Security numbers, as these questions pertain to an application for a child under the age of 18 when applying for an original Social Security card.

      Locate your nearest Social Security office. Call 800-772-1213 and provide them with your Texas zip code or name of the Texas city in which you live. You also can visit their website and enter your Texas zip code into the Social Security office locator search engine, or look in your local phone directory as the Social Security Administration has offices in most major cities across the state of Texas.

      Take your application along with three evidence documents to your local Social Security Administration office. To prove your age you need to provide one of the following pieces of documentation: a birth certificate, religious record prior to your fifth birthday showing your age and date of birth, a passport or your final adoption decree. To prove your identity you must provide evidence of a legal name change (e.g. a marriage license or divorce decree) and the Administration must see your Texas driver’s license, Texas state-issued identity card or your U.S. passport. These evidence documents must be the original documents. You may also mail your application to your local Social Security office in Texas. Should you choose to mail your application you must still include the original evidence document and the Social Security Administration will return them once they have processed your application.
