Contact the relevant Registry of Births, Deaths and Marriages in your state. You can do this online or you can go to any number of regional offices of the Registry.

Download and print the application form or request the Birth Certificate Application Form from a clerk at the office.

Complete the Birth Certificate Application Form, and either mail it to the head of the State’s Registry of Births, Deaths and Marriages, hand it to the desk if you are at an office, or turn it in to a county courthouse, registry office or Government Access Centre. This must be accompanied by your identification; photocopies are acceptable.

In-office submissions can collect the certificate the same day for urgent orders or the following work day for non-urgent orders. Submissions via mail or submissions to places other than the Registry of Births, Deaths and Marriages office take three business days for urgent orders and two weeks for non-urgent orders.

Tips & Warnings

  • For parents of the person named on the birth certificate, or for the person named on the certificate a healthcare card, passport and driver’s license is sufficient identification.

  • For anyone other than the parent or person named on a certificate a letter from the parent or person on the certificate, three forms of their identifications and three forms of your own identification are required.

  • For registering a new birth, send in the Birth Registration Statement given to you at the hospital to the relevant state’s Registry office within 60 days of the birth. The certificate will be sent to you within two weeks.

  • A standard non-urgent birth certificate will cost $44 as of April 2010. Urgent deliveries cost $65. Registering a birth is free.
