Print a Social Security application (see Resources) or call the Social Security Administration's help line at (800) 772-1213 to request that the application be mailed to you, or go to your local Social Security office and request one directly.

Read the instructions carefully and complete the application in blue or black ink and gather your evidence documentation. If you are mailing the documentation, you will need to obtain copies of the information that are certified by the custodian of record. If you will take the information to the office, along with your completed application, bring the originals. Photocopies are not acceptable.
