3. Enclose the $10 application fee in the form of a check or money order. Kentucky Cabinet for Health and Family Services requires that you make the check or money order payable to "Kentucky State Treasurer."

4. Include the address where the birth certificate is to be mailed. There is a specific section at the bottom of the application for this purpose. This section must be completed for all orders.
