How Do I Get a Copy of a New York Death Certificate?
The New York State Department of Health can issue certified copies of death certificates for all deaths in New York State except those in New York City. The New York City Department of Health and Mental Hygiene controls issuing death certificate copies for all five boroughs. In the event that you want to obtain a certified copy of a New York death certificate, you must file a valid request for the copy and prove that you have both the right and need for the copy through positive identification and substantiating documentation.

When the Deceased's Records are in New York City Determine your eligibility for obtaining a copy of the death certificate. If you are not a member of the deceased's immediate family, you must provide notarized documentation that clearly states your reason for needing the certificate.

Gather Positive Identification Assemble your positive identification. This identification must consist of one valid and unexpired photo ID (driver's license, passport, state-issued photo or military photo ID) or a combination of utility or phone bills addressed to you and a letter from any government agency addressed to you and dated within six months of the date you are requesting the death certificate copy.
