An NPI (National Provider Identifier) is a number that is assigned to health care providers to help simplify electronic HIPAA (Health Insurance Portability & Accountability Act) transactions and to help with the coordination of benefit transactions. An NPI can be obtained by individual health care providers as well as organizations. Once you have obtained an NPI, you should keep the data associated with your NPI current. You can update your NPI information by mail or online.

      Visit the NPPES (National Plan & Provider Enumeration System) website (see link in "References").

      Click on the link labeled "NPI Application/Update Form."

      Select the link labeled "Leave The NPPES Website Now." By clicking this link, the NPI update form will open in a separate window.

      Complete the NPI update form by typing in your information. You can also choose to print the form first and complete it by pen. However, typing your information will appear more professional. Note that the editable form fields are highlighted in blue.

      In "Section 1:A," be sure to select your reason for submitting the form, and complete whatever section of the form that needs to be updated. Do not complete all of the form fields. For example, if the only change to your NPI information is an address change, then you only need to provide the new address. If you are deactivating/reactivating your NPI, simply enter your NPI number and your reason for deactivating/reactivating.

      Sign and date the "Certification Statement" that is located in section 4 of the form. The certification statement simply says that you are aware of the necessary requirements to request and maintain your NPI.

      Print your completed form by clicking the "Printer" icon that is located at the top of the page.

      Mail the completed form to the following address:

      Telephone the NPI Enumerator at 800-465-3203 to ensure that your NPI information has been updated. It can take up to 15 business days for your update request to be processed.

      Visit the NPPES website (see link in "References").

      Click on the link labeled "Create Login To View/Update Your NPI Data."

      Complete the online registration form. You will have to enter your NPI number, name/organization name, date of birth, and social security number or EIN number. During the registration process, you will be able to create a user ID and password to log in and access your NPI data.

      Click on the option to "View/Modify NPI Data." Correct the information that needs to be changed and click "Submit."

      Log back into your online account to ensure that your updates have been made. It can take up to 15 business days for your updates to take effect. If after 15 days your data has not been updated, call 800-465-3203 to speak with the NPI Enumerator.
