Instructions

    • 1

Visit a bookstore or stationery store in your area of Oregon that sells legal forms specific to your county as the county courthouse does not offer free forms. Alternatively, you can pay an attorney to draw up the forms for you.

    • 2

Fill out the “Petition for Name Change” form in the presence of the clerk at your county courthouse so it can be notarized, or complete it at home and sign it in the presence of a notary. The Oregon Blue Book website (see Resources) has a list by county of the Oregon county courthouse locations. Use it if you are not sure where your county courthouse is located.

    • 3

Pay the filing fee when you return the completed form to the clerk at your county courthouse. As of March 2010, the filing fee is $158. Also complete the “Notice of Change of Name Hearing” form when you pay the filing fee and submit your “Petition for Name Change” form. On the “Notice of Change of Name Hearing” form select a date 14 days in the future for your hearing. There is also a $41 filing fee for this form. Post a copy of the notice in a public place for 14 days as required by law.

Return to the county courthouse on your hearing date before court is in session. Check with the clerk for your arrival time. Bring with you the “Affidavit - Proof of Posting Notice of Hearing” form with a copy of “Notice of Change of Name Hearing” form attached to it. The clerk will notarize it so you can take it to your hearing.

Wait for the judge to sign your name change form at your hearing. Once he does and you are dismissed, take the form to be filed with the court clerk. You have to do this for the name change to legally go into effect. There is no fee to file the form.

Post a “Notice of Change of Name Decree” stating your name change in a public place, such as a newspaper, for 14 days. Afterward, file “Affidavit - Proof of Posting Notice of Name Change” with the Notice of Change of Name Decree attached to it at the county clerk to be notarized for filing. There is no fee for this filing.
