How to Create a PDF From a Document Image Writer
PDF, which stands for Portable Document Format, is one of the most popular document standards in use. Adobe Acrobat, the most popular PDF reader, is available on many computers, and even the Kindle supports PDF. Microsoft Document Imaging Format (MDI) is a file format used to save a printout of pages when a printer is not available. The virtual printer saves each page to a page in the MDI file, which can be printed out later at the user's convenience.

Take a look at Cute PDF, a powerful and free program that allows you to print almost anything to PDF. You simply open your MDI, Print it, and in the Print Dialogue Box, choose CutePDF as the printer. Click Print and CutePDF will create a PDF.

Check out MDI to PDF, a simple, free software program that allows you to convert MDI to PDF. The free, LIGHT version allows you to open the MDI file to view it, and then save the file as a PDF or JPEG image. You can also simply right click on the MDI file in explorer and there will be an option to "Export MID to..." A dialogue box will open; select PDF and it will quickly convert your file.
