If your name has changed due to marriage, a court order or was printed in error on your passport, you need to change your name on your U.S. passport. If your passport was issued less than a year ago make the adjustment by completing form DS-5504. There is no fee for this service unless you require expedited service. Those that have had their passport for more than a year need to complete form DS-82 and pay applicable renewal fees.

      Complete form DS-5504. This form can be found online at the U.S. Department of State website.

      Take two passport-style photographs. The photographs must measure 2 inches wide by 2 inches tall, be identical and in color. They must also have been taken within the past six months and show your current appearance. The U.S. Department of State has a passport acceptance facility search tool on its website that allows individuals to search for acceptance facilities in their area that takes photographs on-site.

      Mail your application and supporting documents to the address listed on form DS-5504. Include your completed application, a certified copy of your marriage certificate or government-issued document showing your legal name change, your U.S. passport and two recent photographs. You may want to use a traceable method of delivery to confirm the delivery of your application.

      Track your application status online. It takes approximately five to seven business days for your passport application to show online.

    Passport Issued More Than A Year Ago

Tips & Warnings

  • Those that cannot legally show evidence of a name change must apply for a name change in person. Applicants must bring a completed DS-11 form, a valid U.S. passport, two passport photographs and at least three public records showing your date and place of birth and show exclusive use of your assumed name for at least 5 years.
