Request a birth certificate from Georgia's Office of Vital Records.

If you need to replace a misplaced or worn-out Georgia birth certificate, you may do so through Georgia's Office of Vital Records. Only the person listed on the birth certificate, immediate relatives or an agent can apply. You or an eligible person may obtain a birth certificate in person or through the mail. Birth certificate copies are available for a small search fee.

      Generate a request form from the Georgia Office of Vital Records website, listed in the references section.

      Input the full birth name, gender, date and place of birth, and parental information of yourself or the person listed on the birth certificate. Indicate how many birth certificates you need and your mailing address.

      Print out this form. Sign and date it. A PDF copy of this form is also available for download through the website (see Resources). If you do not have access to a printer, you may write this information on a separate piece of paper. Refer to the PDF copy to make sure you have included all the information you will need.

      Provide a photo ID. If you are mailing your application, produce a clear, easy-to-read photocopy of your ID. The Georgia Office of Vital Records website states that a driver's license, state ID or employee ID are valid forms of identification. If you need to provide an alternative form of identification, call the office at 404-679-4702 between the hours of 8 a.m. and 5 p.m. or email phvitalrecords@ gdph.state.ga.us to find out if what you have is acceptable.

      Write a certified check or money order made payable to Vital Records Services. As of 2010, the search fee is $15. Extra copies are $5 each. The office does not accept cash or credit cards.

      Mail your application materials to Vital Records, 2600 Skyland Drive NE, Atlanta, GA 30319. Apply in person at the same address. The vital records office is open Monday through Friday, 8 a.m. to 4:45 p.m. Alternatively, visit the vital records office of the town in which you or the person listed on the birth certificate was born.

Tips & Warnings

  • As of 2010, Georgia's Office of Vital Records has not implemented its own online ordering system. If you wish to request a birth certificate online, you may do so through the Vital Chek Express website. While you will need to pay an additional processing fee, this option is ideal if you need a birth certificate quickly or want to pay with a credit card. Have your birth information and major credit card ready when you order.
