      Download the voter registration form from the Alabama Secretary of State website (see Resources). The form is for use for both first time registrants and already registered voters that need to make a change on their voter registration.

      Complete the voter registration form with the name change. Make any other necessary changes on the form as well. While not required, you can make a copy of photo ID that represents the name change and attach it to the form. Sign and date the bottom of the form.

      Mail or hand deliver your completed voter registration form to your local Board of Registrars. The address for each county Board of Registrars is on page three of the voter registration form. If an election is coming up, be sure to get the form to them more than 10 days before voting day.
