      Download the “Application for Duplicate or Lost in Transit/Reassignment for a Motor Vehicle, Mobile Home or Vessel Title Certificate” form from the Florida Department of Highway Safety and Motor Vehicles (FLHSMV) website.

      Select on the form that you need a duplicate title because the original was lost. Enter in your name, or the owner's if you are not the owner, and contact details. List the details of the car, such as VIN, year, make, model and registration information.

      Take the completed form along with your driver's license and fee payment to your local FLHSMV office to get a replacement title. As of March 2010, the fee is $75.25. The FLHSMV has a listing of all of the office locations if you are unsure of where your nearest location is.
