How to Write a Hospital Patient Survey

How to Write a Hospital Patient Survey thumbnail
To figure out how your patients really feel, consider asking them to fill out a survey.

In a hospital, it may be difficult to treat thousands of patients and also to really know how they feel about the care they are receiving. Give patients at your hospital an anonymous survey that allows them to express how they feel about service and care in several different categories. Another option is to assign a volunteer to visit patients in their rooms and conduct "mini interviews."

Instructions

    • 1

Develop a short series of questions--between five and 10--that touches on the care the patient has received. Some examples from Choctaw Nation Health Services Authority: "How would you rate the service you received?," "Were you treated with courtesy and respect?," and "Did you understand the instructions given to you about your care?"

    • 2

Type out the questions in a Word document on your computer along with answer choices that match the questions. Examples might be: Poor, Unacceptable, Good, Excellent, Yes and No.

    • 3

Type a few questions that have open-ended answers. This will allow the patient to express himself freely and touch on specifics that are not revealed through pre-set answers. Print copies of the survey and distribute to patients.

    • 4

Give the patient an opportunity to submit the survey anonymously or with her name and contact information. Another option is where a patient survey volunteer can assist. If you assign a volunteer to conduct the survey, then you will be able to let the patient know that the volunteer is acting as an advocate for the patient and will make sure the patient's concerns are heard and that someone follows up.

Tips & Warnings

  • Another option is to make the survey available on line.
