Making an amendment to an Idaho birth certificate must be done in writing.

There is no standard approval process for amendments to Idaho birth certificates. The Idaho Department of Health and Welfare will make allowances for amendments to the name on a birth certificate; however, other amendments, such as to paternity or to gender, are not typically allowed.

      Prepare a letter requesting the amendment to be made. The letter should include any supporting documentation available, as well as a copy of the requestor's government-issued photo identification.

      Send the letter along with a completed birth certificate application to the Department of Health and Welfare at the address below. Include a check or money order for the appropriate fee ($13 for one certified copy of the birth certificate, as of February 2010).

      Allow two to three weeks for applications that require amendments to be made to the birth certificate. If your amendment is rejected, you may have to follow up with the Bureau of Vital Records by phone.

Tips & Warnings

  • Although it is possible to submit a birth certificate online through Idaho's VitalChek system, for applications where an amendment is requested this is not recommended. Amendment requests must be made in writing and the VitalChek system does not support this functionality.
