How to Get a Legal Name Change in Hawaii
People change their names for various reasons, although it is most often due to marriage. The laws for changing a name vary from state to state. If you are seeking a name change in the state of Hawaii, you will need the approval of the Office of the Lieutenant Governor. The process involves filling out the appropriate form, paying fees and publishing a notice in a local newspaper.

Download the appropriate form from the website for the Hawaii lieutenant governor's office, at Hawaii.gov/ltgov.aspx/office/name. The following forms are available: Form A (Name Change of Individual), Form B (Name Change of Husband and Wife), Form C (Name Change of Minor by Parents), Form D (Name Change of Husband, Wife and Minor Children), Form E (Name Change of Minor by One Parent with Notarized Consent of the Other) and Form F (Name Change of Parent and Minor Children with the Notarized Consent of the Other Parent).

Complete the form by typing in all information. The form must be typed.

Mail the completed form, with a cashier's check or money order for the filing fee ($50, as of 2010), to: Office of the Lieutenant Governor; P.O. Box 3226; Honolulu, Hawaii, 96801. You can also submit the form in person at: Office of the Lieutenant Governor, State Capitol; 415 S. Beretania St., 5th Floor; Honolulu, Hawaii, 96813. If you submit in person, you can pay cash.

Wait for a Notice of Change of Name from the Office of the Lieutenant Governor. The office will send this notice after the lieutenant governor has signed it.

Use the Notice to publish your change of name in a local newspaper. This must be done within 60 days of the lieutenant governor's signature on the Notice. Show your signed Notice at the newspaper's editorial office and pay the publication fee. The newspaper will send an affidavit to you and to the Office of the Lieutenant Governor as a confirmation that your name change has been published.

Wait for the lieutenant governor to sign the Name Change Order for the name change. This Order and four copies will be sent to you. Send the original to the Bureau of Conveyances, along with the appropriate filing fee ($30, as of 2010).

Change the name on your birth or marriage certificate if you were born or married in Hawaii. Submit your Name Change Order in person or by mail to:
