The Center For Medicaid and Medicare service mandates that physicians obtain a National Provider Identifier (NPI). Luckily, the application process for a NPI is very simple, and can be completed in several ways; the easiest and quickest of which being an online application. Doctors applying for an NPI must have certain information readily available, such as their current provider identification number, UPIN number, taxonomy codes, license numbers, mailing address and their practice address.

Instructions

      Download the online application. The easiest site to use is the National Plan and Provider Enumeration System government website.

      Fill out all the necessary information in the application.

      Send in the application electronically.

    • 4

      Fill out the written application if you do not want to apply for the NPI online. Written applications can be obtained by writing to a national organization, such as the AAFT or NPPES. Double-check all written information for accuracy before mailing it.
