How to Legally Change the Name on a US Passport
United States citizens must have a passport to travel to other countries. Your passport must use your legal name. If you get married, divorced or change your legal name for any other reason, you will need to change your name on your passport.
Things You'll Need
- Printer with black ink
- Government-issued documentation of name change
- Two recent color photographs
- Secure packaging material

Visit the U.S. Department of State's website at http://travel.state.gov/passport/forms/ds5504/ds5504_2663.html.

Fill out Form DS-5504 online and print it in black ink. You can alternatively print the form in black ink and then write on it in black pen.

Mail Form DS-5504 to the address given on the form, along with your passport, two recent color photographs and legal certification of your name change, such as a certified copy of your marriage certificate.

Tips & Warnings
If you have not legally changed your name, also include three or more public records that include your date and place of birth and any applicable fees. These records must prove exclusive use of your new name for at least five years.
Use a traceable delivery method and send your materials in a secure package to prevent damage.
