How to Add Forms to a Microsoft Outlook Calendar
Through Microsoft Outlook, which is part of the Microsoft Office software suite, users can create their own custom forms to use within the program. When working with the calendar feature in Outlook, for example, you can add new forms and change the way you create and view appointments. Calendar forms can be helpful if your business or organization wants to store different information in the Outlook Calendar.

Launch Microsoft Outlook.

Go to the "Tools" menu at the top of the window, expand the "Forms" sub-menu and choose the "Design a Form" option.

Open the "Look In" drop-down menu and select "Calendar."

Double-click on the "Appointment" entry. A new window will open displaying the default calendar and appointment form.

Highlight the field or label that you want to modify.

Use the toolbar at the top of the window to delete or change the selected field or label.

Drag fields from the "Field Chooser" window into the main layout to add new fields to your form.

Click on the "Publish Form" icon at the top of the window to save your design and begin using the new form in Outlook.
