      Launch Microsoft Outlook.

      Go to the "Tools" menu at the top of the window, expand the "Forms" sub-menu and choose the "Design a Form" option.

      Open the "Look In" drop-down menu and select "Calendar."

      Double-click on the "Appointment" entry. A new window will open displaying the default calendar and appointment form.

      Highlight the field or label that you want to modify.

      Use the toolbar at the top of the window to delete or change the selected field or label.

      Drag fields from the "Field Chooser" window into the main layout to add new fields to your form.
